Case Management provides services to complex or high-risk patients receiving care at Sidra. Although case management is often initiated during the acute inpatient care phase, services are also provided in the outpatient settings to connect Sidra specialized clinics with primary care providers in the community. The case management team works collaboratively with the broader health care team to minimize inpatient length of stay by coordinating patient care across the continuum for patients and families with complex care transition needs. Clinical research focused on education, quality improvements and a culture of patient and family safety will be used to inform and guide best practice standards to assure safe, cost-effective and quality outcomes and contribute to Sidra’s reputation as a high reliability organization.



The Clinical Nurse Leader – Case Management supports the Clinical Nurse Manager – Case Management and Homecare in provision of family centered support services to women, children and families to ensure safe and effective transition of services. The primary focus of the Clinical Nurse Leader – Case Management is responsible for management and oversight of care for the medically complex or at-risk patient. The incumbent is to use a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet an individual’s and family’s comprehensive health needs through proper communication. Clinical Nurse Leader – Case Management maintains a comprehensive plan of care coordinating assessments, responding to patient, family and health care member’s inquiries, providing anticipatory guidance and case management, across the health care continuum delegating to the Clinical Nurse – Case Management when appropriate. S/he works in collaboration with other members of the health care team, both within and outside of Sidra and acts as a primary liaison between agencies.

The incumbent delivers clinical care for the majority of their time and supports the Clinical Nurse Manager to ensure successful commissioning. TheClinical Nurse Leader – Case Management is professionally accountable to the Chief Nursing Officer.


  • Makes recommendations to the Manager regarding decisions that have the potential to impact on Case Management or Homecare departments.

  • Makes collaborative decisions related to the overall plans of care for women/children.

  • Makes assessment of financial conditions and requirements of patients and collaborates the Social Workers and/or Financial counselors to try and help the patient and family to fulfill such needs.

  • Arranges medically necessary equipment for discharge and/or refers patients to Homecare or private nursing services for care post discharge.

  • Provides shift supervision and guidance to staff in collaboration with Clinical Nurse Manager and Clinical Nurse Educator.

  • Delivers nursing care to patients and rounds to support efficient patient pathways.

  • Makes day to day decisions about staff allocations during shift to ensure that appropriate staff are allocated to the correct patients to promote safe patient care and the best learning environment.

  • Assumes accountability for assessing, planning, implementing and evaluating patient/family needs along with guiding/assisting the Social Worker I and patient care team with the discharge planning for identified patients.

  • Acts as a liaison, between, the interdisciplinary team, community health and educational services and agencies to clarify information, responsibilities and recommendations to ensure the needs of the patient/family are addressed.

  • Establishes a collaborative working relationship with community- based health care and service providers.

  • Collaborates with the multidisciplinary team and/or nursing colleagues on research-oriented opportunities.

  • Evaluates the length of stay of the patient and ensures appropriateness of services provided in order to effectively achieve safe and comprehensive discharge.

  • Collaborates to gather and utilize data to produce results / indicators to facilitate continual performance improvement, and service improvement activities by clinicians.

  • Participates in regular patient care conferences to facilitate more efficient use of resources, suggest best practices to reduce length of stay and provide greater patient and family satisfaction.

  • Guides and assists the Social Worker I to facilitate complex discharge planning with the team, patient, family and community agencies by attending meetings to ensure comprehensive follow-up.

  • Advocates for the woman/child and their family or caregiver for resources within the hospital and the community by assessing financial status and their current requirements.

  • Collaborates with the Clinical Social Worker and other community resources to try and meet the financial and other basic needs of the patient and family as required.

  • Provides individualized patient care as per Qatar Council for Healthcare Practitioners Licensure Scope, the core standards of nursing and midwifery care and leads on clinical practice.

  • Coordinates complex patient care needs in collaboration with other clinical staff.

  • Ensures safe and high quality patient care during shift through appropriate and equitable allocation of work to the team based on patient acuity and skill mix.

  • Produces, coordinates and manages day to day staff assignments for the assigned shift.

  • Role-models patient experience principles, provides service recovery and identifies continuous improvement opportunities as required.

  • Identifies patient care issues and/or ways to improve and positively impact on the quality of care and patient outcomes.

  • Supports the Clinical Nurse Manager in the employee performance appraisal cycle objectively and assists with action plans arising from appraisals as necessary.

  • Contributes to the achievement of effective utilization of resources.

  • Leads Performance Improvement activities on the unit towards the achievement of agreed key performance indicators as assigned.

  • Supports the selection, development and retention of all staff.

  • Identifies cost efficiencies.

  • Adheres to Sidra’s Nursing and Midwifery Standards of Practice.

  • Bachelor Degree in Nursing (4 years)


  • Diploma in Nursing (3 years after post high school)


  • Registered Nurse

Master’s Degree in Nursing


4+ years of recent clinical nursing experience including 2+ years leadership/supervisory/charge experience in specialty area
  • Experience in an Academic Medical Center.                
  • Demonstrated experience with Clinical Information Systems and Hospital Commissioning

Certification and Licensure

  • Valid professional nursing license in country of origin.


  • Meets licensing requirements of the State of Qatar
