DEPARTMENT/BRANCH PROFILE:

The Capacity Management Departments consists of the Bed Management Unit and the Transfer Center Unit. The Bed Management unit ensures that patients who require patient placement shall be assigned a bed and moved to the appropriate location of care in a timely and safe manner.  The vision to ensure that the right patient will be placed in the right location with the right resources at the right time in the care of the right clinician.

The Transfer Center will act as a primary liaison for all transferring patients to/from Sidra Medical and Research Center to other facilities.  It will be the initial point of contact for physicians who desire to transfer patients to or from Sidra.
JOB SUMMARY: Coordinates the safe transfer of patients into and out of the organization to ensure they are placed in the most clinically appropriate bed under the supervision of the Clinical Nurse Manager-Capacity Management.
KEY ROLE ACCOUNTABILITIES:

Coordination of transfers into and out of the organization, facilitating the appropriate communication between clinical teams.

  • To maintain the clinical safety of all patients transferring within from outside of Sidra

  • Performs transfer call center functions in a responsive manner responding to all callers with the aid of a computer database consisting of facility service profiles, approved clinical protocols, established patient care protocols, and healthcare information.

  • Coordinates transfer of emergent, urgent and elective patients to a final destination facility from all potential referral sources via a single call to initiate acceptance and expedite decision making relative to a potential transfer.

  • Facilitates patient throughput to a final destination facility with an ongoing focus on quality and efficiency.

  • Works directly with CNM – Capacity Management to ensure timely transfer, appropriate bed assignment, and provides accurate patient information to assist with the development of an initial plan of care.

  • Performs some functions of the bed manager as delegated and under the supervision of the same eg performing ward rounds to review discharge status of identified patients

  • Collates daily activity figures to inform required reporting functions of capacity management

  • Maintains clinical skills through allocated clinical shifts.

  • Completes all training and is competent in the application of Cerner’s capacity management system

  • Communicates with physicians and outlying facilities regarding all patient transfers, potential bed placements, and assists with coordinating patient movement from one facility to another.

  • Participates in daily bed management meetings and effectively communicates resources and issues to appropriate area of each facility.

  • Supports unit teams with Patient registration processes to free up CNM – Capacity Management to undertake clinical rounds across hospital.

  • Adheres to established policy and procedure and standards of care; escalates issues through the established Chain of Command in a timely manner

  • Maintains confidential information regarding patient consults and referrals, while serving as a liaison for sending and receiving providers and institutions.

  • Collects, tracks, and monitors the appropriate clinical and demographic data to effectively collaborate with the admitting provider to admit patients to inpatient or observation status. Provides timely data entry into the transfer center electronic data base.

QUALIFICATIONS, EXPERIENCE AND SKILLS:

 

ESSENTIAL

PREFERRED

Education

  • Bachelor’s Degree in Nursing

     

  • Registered Nurse

 

Experience

2+ years clinical experience in an acute hospital environment

 

 

Certification and Licensure

  • Current Nursing License from country of origin

     

  • Eligible to be licensed as anurse in Qatar
