Department Profile:

Social Work provides services to neonatal, pediatric and adolescent patients of all specialties, as well as services to the obstetrical and gynecological inpatients. The focus is on acute inpatient care, however services are also provided to specialized outpatient clinics and selected outpatient populations. The social work team works collaboratively with the broader health care team to coordinate patient care across the continuum for patients and families with biopsychosocial needs, assuring quality outcomes.

Job Summary:

The Social Worker I provides professionally supported family centered services to women, children and families to cope with both acute and chronic illnesses. The Social Worker I is responsible for evidenced based assessment of biopsychosocial and spiritual functioning and capacities, counseling for care planning and decision making, clinical social work treatment and intervention under the direction of mentors and Social Work Leadership. This includes supportive counseling through the diagnosis and treatment phases of care, as well as community resource planning and access. The Social Worker I collaborates with many disciplines, programs, departments, and organizations in the accomplishment of these duties.

 

The Social Worker I strives to provide evidence-based care by integrating research with practice. Promotes continuous improvement and helps to generate innovative ideas within social work practice. S/he maintains a commitment to life-long learning and creates, utilizes and/or disseminates new knowledge to patients and their families, public and other internal /external health healthcare professionals. The incumbent is responsible for ensuring that all duties are carried out to the highest possible standard.

 

The Social Worker I is an active member of the Social Work team and is supported in practice through the use of supervision, mentorship, specific skills based training on wide range of subjects within the clinical field.  The incumbent is expected to be learning while practicing, seeking assistance and enhancing foundational competencies of Social Work Practice.

Key Roles & Responsibilities:
       
    • Makes collaborative decisions related to the overall plans of care for women/children.

    • Conducts, in supported practice, comprehensive biopsychosocial and spiritual clinical assessments and design a treatment/intervention plan to support patient care.

      Assessment and Treatment:

    • Conducts comprehensive biopsychosocial clinical assessments by methods such as interviewing the woman/child and family, obtaining relevant information, gathering social data regarding the child patient and family.

    • Creates a case formulation which guides intervention planning in accordance with professional practice standards and clinical policies to provide excellent patient care.

    • Knowledge of and comfortability with child protection issues and relevant policy, procedure and legislation concerning children.  Proven ability to apply social work principles with sensitivity in practice, to inform patients, families, and key professional in order to ensure the highest standard of patient care is achieved.

    • Provides comprehensive clinical counseling with individuals, families and groups by focusing on the child’s and family’s social, emotional and cultural needs, including adjustment to diagnosis and treatment decisions, loss of function, and dealing with emotional and family crises in the context of evidence based care.

    • Ensures timely and complete documentation of all interventions, assessments and consultations.

    • Advocates for the child and their family or caregiver for resources within the hospital and the community by assessing the financial and other biopsychosocial needs and works in coordination with community resources to meet these needs.  Assists, where appropriate, with the submission of charitable applications and associated documentation.

    • Provides total individualized patient care as assigned according to the core standards of patient care, established policies, and evidence based practices.

    • Assumes accountability for assessing, planning, implementing and evaluating biopsychosocial needs as well as discharge planning for each patient, assessing for current and potential problems.

    • Educates the patient/family and other members of the health care team regarding the social work intervention plan.

    • Utilizes a professional practice framework and evidence based care, identifies improvements while minimizing risks creating care that is optimal for our patients and families.

    • Liaises and works within an interprofessional practice model to ensure high quality, holistic, and safe family centered care.

    • Participates in daily meetings and huddles held to discuss new patients, to plan interventions and monitor the progress of the Social Work patient list.

       

      Discharge Planning:

    • Co-Leads and participates in complex discharge planning, including discharge against medical advice with multidisciplinary teams, patients, families and community agencies. Social Work participation in focused meetings to ensure consistency and biopsychosocial follow-up

    • Contributes to decreasing the length of stay/waiting time as per unit specific goal. Works in collaboration with the Social Work department and medical teams to achieve goals.

    • Acts as an advocate by empowering women/children and their families to make informed choices by providing them with information and support and directing to the relevant support services when appropriate.

       

      Additional accountabilities:

    • Supports the woman/child and their family by understanding the nature of treatment and helps families and other relevant caregivers understand issues by providing education, short-term counseling and conducting crisis intervention.

    • Supports the woman/child and their families by assisting in planning and facilitating psycho-educational and counseling groups with Social Work II.

    • Able and willing to cross support all areas of the hospital including outpatient services, Child Protection services and intensive care units.

    • Able to professionally and capably produce concise, informative, well timed case notes and assessments as well as contend with multiple workload priorities.

    • Able to accept supervision, suggestions and directives in relation to the role and tasks assigned to the position

    • Be able to work rotating shifts in order to provide full service coverage to Sidra Medicine. 

    • Able to complete statistical information and clinical reports in accordance with Social Work and Sidra Medicine policy.

    • Performs other related duties as assigned.

    • Maintains confidentiality of patient and family as professionally appropriate.

     

    Teamwork:

    • Identifies and alerts members of the medical team to changes in medical, emotional, and/or mental status of the patient/family.

    • Identifies impairments and refers to other medical specialties when necessary.

    • Liaises with the appropriate social services in the community and provides transfer/discharge information.

    • Reports equipment failures, safety and security issues, and/or reduction of supply inventory to Supervisor - Social Work or designee.

    • Promotes patient satisfaction as a team member by demonstration of extraordinary service.

    • Promotes a safe environment and safe patient care by embracing a culture of safety and identifying areas of risk to Supervisor – Social Work.

    • Complies with policies for personal and patient safety and for the prevention of healthcare associated infections.

    • Contributes to the achievement of cost efficiency or savings for the service.  Maintains an awareness of resource utilization, fiscal responsibility, exercising care in the ordering and use of staff, equipment and materials.

    • Takes a role in service activities that contribute to excellence in outcomes, a positive patient experience and a healthy professional environment. Contributes to the achievement of clinical quality outcome indicators and patient satisfaction.

    • Co-leads service based performance improvement activities and organizational committees as assigned.  Contributes to special projects/assignments linked to social work strategic priorities and professional practice model.

    • Participates in the continued development, implementation, and evaluation of Sidra Policy and Procedures to align with international best practices.

    • Takes a role in the development of programs that seek to measure outcomes and quality, including audit and research studies, under guidance, support and direction of senior staff.

    • Takes a role in the promotion and maintenance of effective communication and teamwork. Treats colleagues, patients, and visitors with dignity and respect at all times.

    • In partnership with Social Work II, facilitate the evaluation of patient resource needs and assist in application for support, charity applications, immigration and repatriation needs.

    • Supports the Social Work on-call system by being available to assist with forensic and complex cases involving liaising with government, police or other medical services.

    • Works within the Social Work roster to provide service to inpatient and outpatient services. 

       

      Community Relationships:

    • Provides consultation to patients/families, staff, community agencies, satellite centers, national and international centers and the media.

    • Participates in community outreach activities e.g. school-based services, homecare, mental health, etc.

       

      Social Work Research:

    • Identifies social work research opportunities and participates in and/or develops and promotes research.

     

    Teaching, mentoring and learning:

    • Understands that the role of a Social Work I includes that of learning and developing as a professional and being exposed to a variety of patients in order to enhance holistic learning

    • A Social Work I is in “professionally supported practice” meaning that in addition to patient service, workers are expected to attend learning modules, mentoring meetings with peers and supervisions with Social Work Leadership. 

    • Takes part in clinical rotations through wards to ensure exposure to a variety of patient needs and knowledge acquisition.

    • Develops and sustains (maintains) own knowledge, clinical skills (mandatory training) and professional (portfolio or profile) awareness. 

    • Obligated to fulfil all requirements to remain eligible for licensing in Qatar.

    • Provides documented evidence of performance and maintenance of skills consistent with position.

    • Participates in new staff/student orientation aimed at developing competence and performance. Assists in maintaining a suitable learning environment and may assist in the mentoring and teaching of Social Work students and new staff to develop their competence.

Essential Requirements:
  • Bachelor’s Degree in Social Work
  • 2+ years relevant clinical experience
  • Registration as Social Worker and Licensure in Country of Origin
  • Member of leading Social Work association in Country of Origin
  • Demonstrated thorough knowledge of Social Work principles and practices
  • Comprehensive knowledge of Social Work theory and practice
  • Demonstrated ability to conduct and document a comprehensive psycho-social assessment
  • Demonstrated ability to provide individual, family and group counseling
  • Knowledge of the psycho-social needs of children/ families with medical, disabilities, and/ or mental health conditions
  • Knowledge of child development theories
  • Knowledge of child protection issues and relevant legislation concerning children
  • Demonstrated knowledge and ability to work with mental health issues, including DSM, and mental health assessment of depression, suicide, and anxiety

WORK ENVIRONMENT THE ROLE OPERATES IN:

  • Work is normally performed in an acute care hospital and ambulatory care setting

  • Physical effort required may include walking more than is typical due to the distances between hospital units.

  • The Social Work department operates 24 hours per day/7 days per week.

  • Social Worker I will work within the Social Work roster to provide service to inpatient and outpatient services.  Shift work is an expected requirement for the position including overnight coverage and/or on-call/re-call.  It is expected that Social Work I share in coverage over holidays and weekends.
