JOB SUMMARY

The Pediatric Nephrology Nurse Specialist is a trained nurse with experience who is an integral member of the Pediatric Nephrology multidisciplinary team and clinical practice who will be contributing to state of the art patient care delivery.  S/he will coordinate and develop patient care with other care providers and health team members to provide required care as effectively as possible. The nephrology nurse specialist acts as a patient teacher and advocate, assisting the patient in seeking information, assuring the patient has the opportunity for informed consent for treatment decisions, and promoting the maximal level of patient-desired independence. The coordinator will serve as a focal point for the patients to help facilitate all their care needs across the medical system and outside the medical system. S/he is a clinician who has high level critical thinking and works independently to synthesize current principles and concepts in pediatric rheumatology to solve complex problems. 

KEY ROLE ACCOUNTABILITIES:
  • Makes recommendations to the Division Chief regarding decisions that have a unit/service impact

  • Makes day to day decisions regarding unit operations that impact staff

  • Makes recommendations for strategies to improve the nurturing environment for staff

  • Makes day to day decisions about staff allocations to ensure the appropriate staff are allocated to the correct patients to promote the best learning environment

  • Makes decisions about care pathways and service provision to ensure the highest clinical outcomes for patients  

  • High level supervision of clinic flow and smooth operation of clinics in coordination with the CNL, nursing staff and scheduling staff regarding urgent new patient add on, ED consults and follow up patients

  • Provides patient and family teaching related to clinical diagnosis, medications use and adherence in the outpatient setting

  • In office teaching of Intra muscular injections/Medications to be done at home, common side effects, and individual care plan making to include: when to hold medications and call physician/nurse, what to do for fevers in an immunosuppressed patient etc. 

  • Assists with patient phone calls regarding concerns about illnesses, medications side effects, refills, etc.

  • Follow up calls for lab and diagnostic results, simple to complex diagnosis discussions and education based on experience and supervision by physician

  • Responds to patient and family inquiries regarding changes in the patient condition from parent phone calls regarding inpatient, emergency and outpatient visits. Follow up with the attending physician

  • Initiates and coordinates assessments, responds to patient, family and health care member’s inquiries, providing anticipatory guidance, and liaises with other support diagnostic/health services in coordinating patient referrals

  • Communicates with allied health professionals (social worker, transplant Coordinator and dieticians) to provide background and facilitate care

  • Provides patient/family support by ensuring they receive information regarding appropriate and available community health and educational resources

  • Initiates the development of patient friendly resources

  • Collaborates with the health care team and patient/family in developing the plan of care

  • Alerts and update the physicians with any concerns

  • Communicates with capacity management and patient on elective inpatient admissions

  • Assists in clinic procedure preparation of medications, syringes, needles and specimens for joint injections in non-sedated patients done in the clinic.  Education on care post procedure and anticipation of potential problems

  • Demonstrates expert clinical knowledge and skills for the specialized patient group, as per where they are located

  • Nurse Specialist will be expected to be with the physicians and patients in the room to learn about patient and diagnosis

  • Participates in regular patient care conferences to facilitate more efficient use of resources, suggest best practices to reduce length of stay and provide greater patient and family satisfaction

  • Refers to other medical specialties when necessary for optimal patient and family care

  • Educates the patient/family and other members of the health care team regarding the diagnosis, care plan, and education provided to the patient and family

  • Liaises and works within an inter-professional practice team to ensure high quality, holistic, and safe family centered care

  • Empowers patients/families to make informed choices and attain self-management capabilities by providing them with information, support and directing to the relevant clinical services when appropriate

  • Documents all relevant information correctly, appropriately and in a timely manner

  • Accepts accountability for increased responsibility for own professional judgement, actions, outcomes of care and continued competence in accordance with scope of practice, relevant acts and regulations

Inpatient Duties

  • When assigned, rounds with inpatient Nephrology team and meets with patient and families during inpatient admissions to facilitate care

  • Provides patient and family teaching related to clinical diagnosis, medications use and adherence in the outpatient setting

  • Assumes accountability for assessing, planning, implementing and evaluating patient/family needs along with guiding/assisting the Social Worker and patient care team with the discharge planning for identified patients

  • Evaluates the length of stay of the patient with the Nephrology team and ensures appropriateness of services provided in order to effectively achieve safe and comprehensive discharge

  • Provides total individualized patient care according to the core standards of patient care, established policies and procedures and evidence based practices

Research and Quality

  • Participates with development and programmatic support to achieve division Quality Improvement goals and metrics

  • Collaborates and initiates projects with the multidisciplinary team on research-oriented opportunities

  • Collaborates to gather and utilize data to produce results / indicators to facilitate continual performance improvement, and service improvement activities by clinicians

  • Produces reports and data as required to document and monitor patient responses to interventions and education

  • Demonstrates an understanding of evidence-based healthcare methodology and its’ ability to positively impact patient outcomes

  • Evaluates and ensures that clinical documentation, nursing diagnoses and plans of care are integrated and effective in delivering an individual plan of care

  • Observes clinical care standards provided by staff and encourages self-reflection and peer supervision to improve patient outcomes

  • Utilizes a professional practice framework and evidence based care, by identifying improvements while minimizing risks to provide optimal care for our patients and families

  • Promotes an environment of safe patient care by identifying areas of risk and reporting them to the clinical team

  • Applies problem solving techniques and evidence based solutions to unit/clinic/service related issues. Evaluates and ensures appropriate escalation of issues to meet patient/family/staff needs and resolve conflicts

  • Ensures relevant clinical information is communicated to all staff effectively and efficiently

  • Consults with unit leadership to resolve issues relating to Nephrology patients and communicates effectively to maintain positive working relationships and achieve desired outcomes

  • Promotes patient satisfaction as a team member by demonstration of extraordinary service

  • Participates in the continued development, implementation, and evaluation of Sidra Policy and Procedures to align with international best practices

  • Promotes and shares knowledge via mentorship/preceptorship programs in line with relevant research and evidence based practice aimed at developing competence and performance  

  • Actively participates in patient and nursing education programs

  • Develops and sustains (maintains) own knowledge, clinical skills (mandatory training) and professional (portfolio or profile) awareness and maintains a professional profile.  Provides documented evidence of performance and maintenance of skills consistent with position

  • Assists in maintaining a suitable learning environment and acts as a preceptor/mentor in for new staff in the Nephrology clinics to develop their competence

  • Performs other related duties as assigned

QUALIFICATIONS, EXPERIENCE AND SKILLS – SELECTION CRITERIA:

 

ESSENTIAL

PREFERRED

Education

  •  Bachelor’s Degree in Nursing
  • Registered Nurse (3 Year graduation program or equivalent)
  • Master’s Degree in Nephrology related field
                 

 

Experience

  • 4+ years of clinical Nursing experience in an acute hospital setting inclusive of 2+ years of leadership experience in Nephrology
                   

Certification and Licensure

  • Valid professional nursing license in country of origin
  • Meets licensing requirements of the State of Qatar
