Department: The Department of Pharmacy provides world class pharmaceutical care to a diverse patient base of women and children of both local and international origin. Our expert staff is dedicated to the provision of specialized pediatric and maternal healthcare in an empathetic and caring clinical environment.  Pharmacy Department services inpatient, emergency and ambulatory settings in a 399 bed facility utilizing state-of-the-art equipment and technologies.
Job Summary:

TheClinical Pharmacy Specialist is a key member of the Pharmacy clinical care team.  The incumbent leads a team of Clinical Pharmacists to provide highly specialized and complex Clinical Pharmacy services on a daily basis in a specialty clinical area, or various clinical areas, as determined by hospital needs. He/She is responsible for the creation and maintenance of clinical protocols and services in his/her designated specialty area. The Clinical Pharmacy Specialist acts as a drug information and education resource for patients, students, colleagues, Medical and Nursing staff, and other healthcare professionals. He/She serves as a facilitator, enabler and mentor for the Pharmacists on his/her team, but also provides mentorship for other Pharmacy staff, students and residents. As a Clinical Pharmacy team leader, the Clinical Pharmacy Specialist ensures that the clinical competency of his/her Pharmacy team is maintained at a very high level.

Positions available in the following fields: General pediatrics, pediatric critical care, neonatal intensive care, hematology/oncology, women's health, pediatric emergency medicine and pediatric ambulatory care.

 

Key Role Accountabilities:

LEADERSHIP:

  • Leads a team of Clinical Pharmacists to provide highly specialized and complex Clinical Pharmacy services on a daily basis in a specialty clinical area, or various clinical areas, as determined by hospital needs.

  • Provides mentorship for Pharmacy staff, students and residents.

  • Prepares an effective Professional Development Plan (PDP) for each direct report.

  • Provides regular performance feedback to employees and assists them in progressing along development continuum.

  • Empowers, stimulates, encourages and supports staff to work towards departmental goals.

  • Ensures a harmonious, professional working relationship between Clinical Pharmacists and the Medical staff, Nursing and all other healthcare professionals.

  • Monitors and documents the clinical competency of all Clinical Pharmacists within his/her clinical team on an ongoing basis and develops improvement plans when deficiencies are identified.

  • In conjunction with the Pharmacy Management team, responsible for the performance evaluations of Clinical Pharmacists within his/her clinical team.

 

CLINICAL:

  • Responsible for the creation and maintenance of clinical protocols and services in designated specialty area including (but not limited to) Pharmacokinetics consultations, medication history/reconciliation, antimicrobial stewardship and Medication Use Evaluation (MUE).

  • Provides drug information and serves as an education resource for patients, students, colleagues, Medical and Nursing staff, and other healthcare professionals.

  • Fully integrates her/his specialized knowledge of the scientific and clinical use of medications, including medication action, dosing, adverse effects, and drug interactions, when performing their patient care activities in collaboration with other members of the health care team.

  • Participates in patient care rounds.

  • Develops and implements drug level monitoring protocols and modifies processes to ensure optimization and accuracy of the drug monitoring.

  • Participates in drug use evaluation and drug protocol management.

  • Advises the patient on how to best take his or her medications.

  • Ensures continuity of care between the acute and ambulatory patient care environments.

  • Documents all clinical activities and interventions accurately and completely, as per policy and procedure.

  • Assists the Manager of Clinical Pharmacy in addressing clinical matters relating to drug shortages and other drug procurement issues.

  • Provides clinical pharmacotherapeutic expertise in the medical field(s) of his/her specialty area as well as in additional clinical programs as assigned.

  • Participates in projects with the local College of Pharmacy and local Pharmacy organizations as needed.

  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and disease state appropriateness relative to drug and dose.

  • Responsible for drug class reviews and drug monographs to be submitted to the Pharmacy & Therapeutics (P&T) committee for its review.

 

COMPETENCY:

  • Ensures that the  clinical competency of his/her team is maintained at an exceptionally high level.

  • Provides in-service education to Pharmacy and non-Pharmacy personnel.

  • Participates in the teaching and precepting of Pharmacy students and residents.

  • Communicates effectively with healthcare professionals, patients and family members where appropriate.

  • Demonstrates proficiency in the compounding of extemporaneous preparations.

  • Demonstrates proficiency in aseptic technique during the preparation of sterile pharmaceutical products.

  • Has (or will obtain) certification in Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS) and/or Neonatal Resuscitation Program (NRP), and trains with code team and attends code situations, as required by assigned specialty area(s).

  • Maintains professional competency and skills sufficient to meet current and future needs of the facility.

 

MEDICATION SAFETY:

  • Adheres to policies and procedures relating to Medication Safety, and provides suggestions for increasing it.

  • Supports and participates in the goals of the organization's quality improvement, risk management, and patient safety programs to include (but not limited to) National Patient Safety Goals, Core Measures, and other medication-related Joint Commission International (JCI) initiatives.

  • Works with the Medication Safety Officer to design systems and procedures that aim to prevent medication errors and increase medication safety in his/her specialty area.

 

OPERATIONAL:

  • Supervises the safe and competent use of all Pharmacy equipment used in their area of practice.

  • Verifies medication orders for accuracy and clinical appropriateness, checks medications prepared by Pharmacy Technicians for accuracy prior to dispensing and notifies appropriate persons if changes are required.

  • Participates in the orientation and training of Pharmacy students, residents and newly hired Pharmacists and Technicians.

  • Objectively adheres to and enforces policies and procedures.

  • Provides accurate shift-to-shift communication regarding unsolved situations requiring future action.

  • Assists in the acquisition of non-formulary, compassionate use, and investigational medications when needed.

  • Provides professional, courteous customer service to all customer types (patients/family, Nursing, Medical Staff, others).

 

QUALITY IMPROVEMENT:

  • Sits on committees as required to provide recommendations pertaining to medication use and processes.

  • Identifies, designs, and analyzes Medication Use Evaluations (MUE) to promote rational drug therapy and determine action plan based on results.

  • Retrieves clinical information for monitoring and review of the medication use process.

  • Directs Process/Quality Improvement activities of his/her specialty area, including Key Performance Indicators (KPI’s).

  • Routinely monitors and evaluates clinical interventions submitted by Pharmacy staff in his/her specialty area, taking action to address undesirable trends as needed.

 

REGULATORY:

  • Adheres to Sidra’s standards as they appear in the Code of Conduct and Conflict of Interest policies.

  • Complies in all activities with accreditation, legal, regulatory, and safety requirements for a specific practice setting.

  • Completes all required Continuing Education activities to maintain licensure to practice as a Pharmacist in Qatar, per the Qatar Council for Healthcare Practitioners (QCHP).

     

In view of the developing and changing needs and opportunities within Sidra during this start-up phase, this position may be required to perform other duties as assigned and reporting relationships may vary.

Essential Requirements:
  • Doctor of Phamacy (Pharm.D) from an ACPE / or equivalent accredited School of  Pharmacy with Residency, or Board of Pharmacy Specialties (BPS) certification in  field of practice

    OR 

  •  Postgraduate Diploma in Clinical Pharmacy (PGDipClinicPharm)

  • 3+ years’ Clinical Pharmacist experience in a Hospital in North America or equivalent in UK, Republic of Ireland, EU States, South Africa, Australia or New Zealand, after completion of PharmD and Residency program

    OR

  • 6+ years’ Clinical Pharmacist experience in a Hospital in North America or equivalent in UK, Republic of Ireland, EU States, South Africa, Australia or New Zealand, after completion of Postgraduate Diploma in Clinical Pharmacy (PGDipClinicPharm) [and no Residency program]

  • Licensed in Pharmacy in country of origin

  • Obtain State of Qatar Pharmacy License after on boarding 

  • Excellent clinical knowledge as it pertains to medication and disease management

  • Demonstrated ability to work with Pharmacy Information Systems

  • Proficiency with Microsoft Office suite

Fluency in written and spoken English
