Department Profile:

The Allied Health Department is a diverse mix of professions and services working together to provide patients with the comprehensive care that they require. Allied Health will provide services to neonatal, pediatric and adolescent patients of all specialties, as well as obstetrical and gynecological patients.

Job Summary:

The Social Worker I provides family centered support services to women, children and families to cope with both acute and chronic illnesses. The Social Worker I is responsible for implementation of continuing care, referral processes, and maintains a strong internal and external customer focus. Social Worker I works collaboratively with social work, case coordinator, other disciplines, programs, departments, and organizations for accomplishment of these duties.

The Social Worker I provides evidence-based patient care by integrating research with practice. Promotes continuous improvement and helps to generate innovative ideas within social work practice. Maintains a commitment to life-long learning and creates, utilizes and/or disseminates new knowledge to patients and their families, the public and other internal /external health professionals.  The incumbent is responsible for ensuring that all duties are carried out to the highest possible standard.

The Social Worker I participates in the provision of teaching of colleagues and mentoring of students on wide range of subjects within the clinical field.

 

Key Roles & Responsibilities:
  • Conducts patient and family needs assessments by methods such as interviewing the woman/child and family, obtaining relevant information, gathering social data regarding the child and family and formulation of discharge plan of care, in accordance with professional practice standards and Sidra policies and procedures
  • Supports the woman/child and their family by understanding the nature and treatment of the presenting issues by providing internal and external referral options for discharge plan of care
  • Participates in complex discharge planning with the team, patient, family and community agencies by coordinating regular meetings to ensure smooth transition of care
  • Advocates for the child and their family or caregiver for resources within the hospital and the community by assessing the financial needs and works in coordination with community resources to meet these needs
  • Identifies, maintains and updates information on referral agencies and resources necessary for continuation of care for patients and families
  • Provides total individualized patient care as assigned according to the core standards of patient care and established policies and evidence based practices
  • Assumes accountability for assessing, planning, implementing and evaluating discharge plan. Assess current and potential discharge planning issues
  • Contributes to decreasing the length of stay/waiting time as per unit specific goal. Works in collaboration with Case Management to achieve the goal
  • Documents all relevant information correctly, appropriately and in a timely manner related to the patient
  • Identifies and alerts members of the medical team to changes in availability of referral services
  • Liaises with the appropriate services in the community and provides transfer/discharge information
  • Educates the patient/family and other members of the health care team regarding the discharge services plan
  • Participates in community outreach activities e.g. school-based services, home care
  • Identifies social work research opportunities and participates in and/or develops and promotes research
  • Disseminates research and clinical findings through publications, presentations, seminars, etc
  • Reports equipment failures, safety and security issues, and/or reduction of supply inventory to Social Work Supervisor or designee
  • Act as an advocate by empowering women/children and their families to make informed choices by providing them with information and support and directing to the relevant support services when appropriate
  • Promotes patient satisfaction as a team member by demonstration of extraordinary service
  • Utilizes a professional practice framework and evidence based care, identifies improvements while minimizing risks creating care that is optimal for Sidra patients and their families
  • Promotes a safe environment and safe patient care by embracing a safety culture and identifies areas of risk to Supervisor - Social Work Services. Complies with policies for personal and patient safety and for the prevention of healthcare associated infections
  • Contributes to the achievement of cost efficiency savings for the service.  Maintains an awareness of resource utilization, exercising care in the ordering and use of staff, equipment and materials
  • Participates in service activities that contribute to excellence in outcomes, a positive patient experience and a healthy professional environment
  • Contributes to the achievement of clinical quality outcome indicators and patient satisfaction
  • Participates in service based performance improvement activities and organizational committees as assigned. Contributes to special projects/assignments linked to social work strategic priorities and professional practice model
  • Participates in the continued development, implementation, and evaluation of Sidra Policy and Procedures to align with international best practices
  • Liaises and works within an inter professional practice model to ensure high quality, holistic, and safe family centered care
  • Promotes and shares knowledge via mentorship/preceptorship programs in line with relevant research and evidence based practice
  • Participates in the development of programs that seek to measure outcomes and quality, including audit and research studies, under guidance, support and direction of senior staff
  • Participates in patient education programs
  • Develops and sustains (maintains) own knowledge, clinical skills (mandatory training) and professional (portfolio or profile) awareness. Provides documented evidence of performance and maintenance of skills consistent with position
  • Participates in new staff/student orientation aimed at developing organization competence. Acts as a preceptor/mentor for Social work students and new staff to enhance their competence
  • Demonstrates transcultural principles in social work practice
  • Performs other related duties as assigned

  In view of the evolving needs and opportunities within Sidra during this pre-operational phase, this position may be required to perform other duties as assigned and reporting relationships may vary.

 

Essential Requirements:
  • Bachelor’s Degree in Social Work
  • 2 years of relevant clinical experience 
  • Registration as Social Worker and Licensure in country of origin
  • Member of leading Social Work association in country of origin
  • Demonstrated thorough knowledge of Social Work principles and practices
  • Demonstrated knowledge of Social Work theory and practice
  • Demonstrated ability to conduct and document a comprehensive discharge needs assessment
  • Knowledge of the developmental and social needs of children/families with medical, disabilities, and/or mental health conditions
  • Knowledge of child protection issues and relevant legislation concerning children
  • Demonstrated knowledge and ability to work with mental health issues
  • Working knowledge of other health care disciplines and their role in client care
  • Demonstrated ability to work collaboratively and effectively in an inter-professional environment
