Department Profile:

The Allied Health Department is a diverse mix of professions and services working together to provide patients with the comprehensive care that they require. Allied Health will provide services to neonatal, pediatric and adolescent patients of all specialties, as well as obstetrical and gynecological patients.

Job Summary:

The Social Worker II provides family centered support services to women, children and families to cope with both acute and chronic illnesses. The Social Worker II is responsible for assessment of psycho-social functioning and capacities, counseling for care planning and decision making, social work treatment, including support through the diagnosis and treatment phases of care, and community resource planning and access. Social Worker II works collaboratively with many disciplines, programs, departments, and organizations in the accomplishment of these duties. The Social Worker II strives to provide evidence-based care by integrating research with practice. Promotes continuous improvement and helps to generate innovative ideas within social work practice. Maintains a commitment to life-long learning and creates, utilizes and/ or disseminates new knowledge to patients and their families, the public and other professionals internal/ external health professionals. The Social Worker II is responsible for ensuring that all duties are carried out to the highest possible standard. The Social Worker II participates in the provision of teaching and supervision of peers on wide range of subjects within the clinical field.

Key Roles & Responsibilities:
  • Conducts comprehensive psycho-social and family assessments by methods such as interviewing the woman/ child and family, obtaining relevant information, gathering social data regarding the child and family and formulation of assessment and plan of intervention, in accordance with professional practice standards and clinical policies
  • Supports the woman/ child and their family by understanding the nature and treatment of the presenting issues by providing education, short-term counseling and crisis intervention to families and other relevant caregivers
  • Provides comprehensive clinical counseling with individuals, families and groups by focusing on the child’s and family’s social, emotional and cultural needs, including adjustment to diagnosis and treatment decisions, loss of functioning and dealing with emotional and family crises in the context of evidence based care
  • Supports the woman/ child and their families by organizing and facilitating psycho-educational and counseling groups
  • Facilitates in complex discharge planning with the team, patient, family and community agencies by coordinating regular meetings to ensure psycho-social follow-up
  • Advocates for the child and their family or caregiver for resources within the hospital and the community by assessing the financial and other psycho-social needs and works in coordination with community resources to meet these needs
  • Provides total individualized patient care as assigned according to the core standards of patient care and established policies and evidence based practices
  • Assumes accountability for assessing, planning, implementing and evaluating psycho-social needs as well as discharge planning for each patient, assessing for current and potential problems
  • Contributes to decreasing the length of stay/ waiting time as per unit specific goal. Works in collaboration with Care Coordination to achieve goals
  • Documents all relevant information correctly, appropriately and in a timely manner
  • Identifies and alerts members of the medical team to changes in medical status of the patient
  • Identifies impairments and refer to other medical specialties when necessary
  • Liaises with the appropriate social services in the community and provides transfer/discharge information
  • Educates the patient/family and other members of the health care team regarding the social work services  plan
  • Provides expert consultation to patients/families, staff, community agencies, satellite centers, national and international centers and the media
  • Participates in community outreach activities e.g. school-based services, home care
  • Identifies social work research opportunities and participates in and/ or develop and promote research
  • Disseminates research and clinical findings through publications, presentations, seminars, etc.
  • Reports equipment failures, safety and security issues, and/or reduction of supply inventory to Social Work services Supervisor or designee
  • Act as an advocate by empowering  women/children and their families to make informed choices by providing them with information and support and directing to the relevant support services when appropriate
  • Promotes patient satisfaction as a team member by demonstration of extraordinary service
  • Utilizing a professional practice framework and evidence based care, identify improvements while minimizing risks creating care that is optimal for our patients and families
  • Promotes a safe environment and safe patient care by embracing a safety culture and identifying areas of risk to Social Work Services Supervisor. Complies with policies for personal and patient safety and for the prevention of healthcare associated infections
  • Contributes to the achievement of cost efficiency savings for the service. Maintains an awareness of resource utilization, exercising care in the ordering and use of staff, equipment and materials
  • Takes a lead role in service activities that contribute to excellence in outcomes, a positive patient experience and a healthy professional environment. Contributes to the achievement of clinical quality outcome indicators and patient satisfaction
  • Leads service based Performance Improvement activities and organizational committees as assigned. Contributes to special projects/ assignments linked to social work strategic priorities and professional practice model
  • Participates in the continued development, implementation, and evaluation of Sidra Policy and Procedures to align with international best practices
  • Liaises and works within an inter-professional practice model to ensure high quality, holistic, and safe family centered care
  • Promotes and shares knowledge via mentorship/ preceptorship programs in line with relevant research and evidence based practice
  • Takes a lead role in the development programs that seek to measure outcomes and quality, including audit and research studies, under guidance, support and direction of senior staff.
  • Actively participates in patient education programs
  • Develops and sustains (maintains) own knowledge, clinical skills (mandatory training) and professional (portfolio or profile) awareness and maintains a professional profile. Provides documented evidence of performance and maintenance of skills consistent with position
  • Participates in new staff/ student orientation aimed at developing competence and performance. Assists in maintaining a suitable learning environment and acts as a preceptor/ mentor in the supervision and teaching of Social Work students and new staff to develop their competence
  • Demonstrates trans-cultural principles in Social Work practice
  • Takes a lead role in the promotion and maintenance of effective communication and teamwork. Treats colleagues, patients, and visitors with dignity and respect at all times
  • Actively welcomes and provides support for new staff/ students to the unit. Contributes to the retention of all staff to meet the service specific turnover rate goal. Participates in retention activities
  • Maintains a professional demeanor at all times. Acts as a role model for junior staff

Essential Requirements:
  • Bachelor’s Degree in Social Work
  • 5 years relevant clinical experience
  • Registration as Social Worker and Licensure in Country of Origin
  • Member of leading Social Work association in Country of Origin
  • Demonstrated thorough knowledge of Social Work principles and practices
  • Comprehensive knowledge of Social Work theory and practice
  • Demonstrated ability to conduct and document a comprehensive psycho-social assessment
  • Demonstrated ability to provide individual, family and group counseling
  • Knowledge of the psycho-social needs of children/ families with medical, disabilities, and/ or mental health conditions
  • Knowledge of child development theories
  • Knowledge of child protection issues and relevant legislation concerning children
  • Demonstrated knowledge and ability to work with mental health issues, including DSM, and mental health assessment of depression, suicide, and anxiety
  • Demonstrated knowledge and ability to work with grief and loss issues
  • Working knowledge of other health care disciplines and their role in client care
